Prerequisites:

For those residents requiring work sponsorship, we will only support a J-1 visa. You must have successful completion of PGY 1 and PGY 2 years in an ACGME accredited Surgery residency program prior to start date.

1. Current CV
2. Updated Personal Statement
3. Documentation of USMLE3 PASS and USMLE1,2 transcripts. (or COMPLEX)
4. Three letters of recommendation from current faculty, including one from your program director. PD letter should state that you are in good standing and have had no probationary events.
5. ABSITE scores for each year of training
6. ACGME case log defined category report.
7. List of Completed rotations
8. ECFMG certification.
9. Medical School transcript
